Food Order Forms

How to use: Print and fill (or fill and print) the order form. Take the printed form to Sarah Kim Fein (Room 600 Fairchild) or Jaya Santosh (Room 500 Fairchild) for authorized signature. Fax or email the signed form to the vendor. Once you have sent your order, please call the phone number at the top of the form to confirm that it has been received.  You must give a copy of this form to Sarah or Ellie in the 600 office along with the yellow or pink invoice provided at the time of delivery.  The invoice will then be processed for payment using the Project number provided.

Once you receive your order/delivery, a copy of this form and the itemized invoice received from V&T must be sent to Inna Boluk ([email protected]) no later than 30 days after the event.

Tip will be 10% of the total, included automatically. Contact Anthony at 212-666-8051 with any changes.
