Listed below are links to various forms, which you might find useful. Some forms are fillable PDFs: please download the form, fill it out electronically in Adobe if possible, save, and send by email.

  • Application for MA degree - GSAS requires that the MA be completed by the end of the Second year. Second years should fill out the MA application in SSOL under "Degree App Status" by December 1. - MA requirements
     
  • Qualifying Exam Form (PDF) - Summer semester of the Second year, Deadline: June 30th
    Note:
    If the meeting is held remotely on Zoom, the chair of the committee may sign electronically for other committee members.
     
  • Committee Meeting Form (PDF) for 2nd, 3rd, 4th years - 6 months after passing the Qualifying Exam, then annually
    Note:
    If the meeting is held remotely on Zoom, the chair of the committee may sign electronically for other committee members.
  • Committee Meeting Form for 5th + years (PDF) - Every 6 months starting in the 5th year
    Note:
    If the meeting is held remotely on Zoom, the chair of the committee may sign electronically for other committee members.

  • Application for M.Phil. - Third years may apply if requirements are completed. GSAS requires that the MPhil be completed by the end of fourth year.
    Note:
    All three forms should be filled to apply for the Master of Philosophy.  Please use Adobe Acrobat to fill the forms, NOT a web browser.
     
  • Application for the Dissertation Defense You are highly encouraged to elect to embargo your dissertation for a period of one or two years from the date of your deposit. Discuss it with your faculty advisor.  Please see more about electronic deposit and the option to embargo here on the GSAS Dissertation Office Electronic Deposit Gateway page.
    • Columbia University has partnered with Turnitin  to assist University faculty and staff with online grading and plagiarism prevention. Access to this service is at no cost to University faculty and staff. There are three access levels in Turnitin: Administrator, Instructor, and Student.
      • Administrators may: Create instructor accounts and courses and review usage statistics across courses
      • Instructors may: Create assignments, set parameters for students to upload document for plagiarism detection and monitor originality report for student submission (TAs requesting access for a course must obtain the approval of the course instructor)
      • Students may: Upload documents for a course to check for plagiarism
